
UKK Health Screening – Form for Evaluating the Safety and Feasibility of Fitness Testing

The UKK Institute’s Health Screening Form enables the fitness professional to evaluate a participant’s health status prior to conducting fitness tests.
The participant’s health status and physical activity habits should be considered when selecting test methods. Background information relevant to test safety can be assessed in a way that is appropriate for the specific tests being used and the participant’s target group. One tool for collecting background information is the UKK Institute’s Health Screening Form.
The UKK Health Screening Form provides a baseline assessment that is essential for ensuring test safety before conducting fitness tests. The form may be customized and supplemented with extra questions if required. Factors such as the target group, selected tests, aim of the testing, and any potential need for physical activity counselling or lifestyle guidance may require additional or differently phrased questions.
The Health Screening Form was updated in 2024 in collaboration with the UKK Institute and the Tampere Research Center of Sports Medicine. The English version of the form was published 30.3.2026
