Fall Risk Assessment – version 1

Kuvaus

The Fall Risk Assessment included in the KaatumisSeula tools is intended for a rough fall risk assessment. The form is based on self-assessment and the purposed for the elderly to fill out the form themselves.

There are two different versions of the assessment form according to use:

• Version 1 – for members of organizations and associations (the form does not contain the name of the person filling in)
• Version 2 – for social and healthcare professionals (the form also includes the name of the person filling in)

Version 1 is intended for use in various group activities organized by organizations and associations and when there is no possibility of personal counselling when filling out the form. Use the instructions for filling out the form.
