Fall Risk Assessment – version 2

Kuvaus

The Fall Risk Assessment included in the KaatumisSeula tools is intended for a rough fall risk assessment. The form is based on self-assessment and purposed for the elderly to fill out the form themselves.

There are two different versions of the assessment form according to use:

• Version 1 – for members of organizations and associations (the form does not contain the name of the person filling in)
• Version 2 – for social and healthcare professionals (the form also includes the name of the person filling in)

Version 2 is intended to be used when there is an opportunity for personal counselling in connection with the filling. Use the instructions for filling out the form.
